What You’ll Do
- Provide in-home respiratory assessments, treatment, and follow-up care for patients with COPD, asthma, CHF, ventilator or tracheostomy needs, and other chronic respiratory conditions.
- Educate patients and caregivers on equipment operation, oxygen safety, and preventive strategies.
- Partner closely with nurses, physicians, case managers, and family to coordinate seamless transitions from hospital to home.
- Monitor patient progress, report changes promptly, and support physician-directed Plan of Care/Treatment for optimal outcomes.
- Serve as a trusted advocate and motivator — empowering patients to take control of their health journey.
What You’ll Bring
- Current GA Respiratory Therapist license (RRT or CRT) and BLS certification.
- Previous experience in home health, subacute, or pulmonary rehab is preferred.
- The ability to supervise clinical personnel and have a basic understanding of insurance requirements (Medicare, Medicaid, and private insurance).
- Comfortable working independently while maintaining strong collaboration with the care team.
- A genuine passion for meeting patients where they are — literally and emotionally.
